ABSTRACT

As epicondilalgias medial e lateral são as causas mais frequentes de sintomas dolorosos nos membros superiores. Observa-se um aumento no número de publicações sobre os efeitos do treino da musculatura escapular no manejo clínico das epicondilalgias, porém desconhecemos uma revisão sistemática que avalie os efeitos do treinamento resistido nos músculos escapulares nessa condição. Objetivo: revisar sistematicamente os efeitos do treinamento resistido nos músculos escapulares nas epicondilalgias. Metodologia: foi realizada uma busca científica (SciELO, PubMed, LILASCS, PEDro) de estudos clínicos controlados sobre os efeitos do treinamento resistido nos músculos escapulares nas epicondilalgias publicados até maio 2021. A escala de PEDro foi aplicada para avaliação da qualidade metodológica. Resultados: foram encontrados 266 artigos e 3 foram incluídos na revisão sistemática. Os estudos inclusos enfatizaram o treinamento dos músculos trapézio médio, trapézio inferior e serrátil anterior associados a reabilitação convencional. Através da análise individual dos estudos observa-se efeitos positivos do treino escapular na dor, na função (Patient Rated Tennis Elbow Evaluation Questionnaire ­ PRTEE) e na força de preensão manual em indivíduos com epicondilalgias lateral. A metanálise revela que adicionar exercícios escapulares a fisioterapia convencional reduz a dor (EVA 0-10) em 1.23 (95% IC, 2,00-0,47, p=0,002, I2=47%, p para heterogeidade = 0.17) e aumenta a função (PRTEE 0-100) em 5.47 (95% IC, 10,00-0,93, p=0,02, I2=0%, p para heterogeidade = 0.84) na epicondilalgias lateral. A média de pontuação da escala PEDro foi de 6,33/10. Conclusão: o treinamento resistido dos músculos escapulares associados a reabilitação reduzem dor e aumentam função em indivíduos com epicondilalgia lateral. Sugere-se a realização de mais estudos com melhor qualidade metodológica para avaliar a importância do fortalecimento dos músculos escapulares em indivíduos com epicondilalgias. (AU)


Medial and lateral epicondylalgia are the most requente causes of painful symptoms in the upper limbs. There is na increase in the number of publications on the effects of scapular muscle training in the clinical management of epicondylalgia, but we do not know of a systematic review that assesses the effects of resistance training on scapular muscles in this condition. The aim of the study was to systematically review the effects of resistance training on scapular muscles in epicondylalgia. A scientific search (SciELO, PubMed, LILASCS, PEDro) of controlled clinical studies on the effects of resistance training for the scapular muscles in epicondylagias published until May 2021 was performed. The PEDro scale was applied to assess the methodological quality. 266 articles were found and 3 were included in the systematic review. The included studies emphasized the training of the middle trapezius, lower trapezius and serratus anterior muscles associated with conventional rehabilitation. Through the individual analysis of the studies, positive effects of scapula training on pain, function Patient Rated Tennis Elbow Evaluation Questionnaire ­ PRTEE) and handgrip strength were observed in individuals with lateral epicondylalgia. The meta-analysis reveals that adding scapular exercises to conventional physical therapy reduces pain (VAS 0-10) by 1.23 (95% CI, 2.00-0.47, p=0.002, I2=47%, p for heterogeneity = 0.17) and increases function (PRETEE 0-100) by 5.47 (95% CI, 10.00- 0.93, p=0.02 I2=0%, p for heterogeneity = 0.84) in lateral epicondylalgia. The average score of the studies on the PEDro scal e was 6.33 out 10. In conclusion, resistance training of scapular muscles associated with rehabilitation reduces pain the increases function in individuals with lateral apicondylalgia. It is suggested that more studies be carried out with better methodological quality to assess the importance of strengthening the scapular muscles in individuals with epicondylalgia. (AU)

ABSTRACT

Resumen La valoración médico legal implica establecer un nexo de causalidad entre lo denunciado por los usuarios y los hallazgos encontrados clínica y radiológicamente, esto implica tener un conocimiento amplio de la biomecánica que permita interrelacionar de manera objetiva un mecanismo de trauma que se relacione en su génesis con el movimiento denunciado. En este artículo se realizará una revisión bibliográfica de la epicondilitis, tomando en consideración todos los aspectos de importancia médico legal.


Abstract The forensic evaluation implies establishing the causality between the story and the clinical and radiologic findings, this implies having a broad knowledge of biomechanics that allows an objective interrelation of a trauma mechanism. This article consists in a bibliographical revision about the etiopathogenesis of the epicondylitis and the interpretation of this patology in relation to medico legal analysis.

OBJECTIVE@#To quantitatively study the biomechanical parameters of Bachuorounian manipulation in the treatment of humeral epicondylitis, and discuss the effects of individual characteristics on the biomechanical parameters were discussed.@*METHODS@#From July 2019 to February 2020, 40 patients with external humeral epicondylitis were selected, including 18 males and 22 females, ranging in age from 20 to 50 years old, with an average of (34.37±8.41) years old;and the course of disease ranged from 1 to 11 months, with a mean of (6.05±2.71) months. The biomechanical parameters of the elbow joint of the affected side were measured by using the biomechanical sensor. At thesame time, the individual characteristic parameters of patients were collected to analyze the influence of different individual characteristics of patients on biomechanical parameters.@*RESULTS@#The results of mechanical analysis in each stage of the bachuorounian manipulation were as follows:the rolling back rotation force was (31.17±2.99) N;the buckling bending drawing force was (44.99±2.38) N;the rolling pre rotation force was (31.03±2.75) N;and stretching drawing force was (48.75±2.09) N. The correlation analysis between the parameters showed that there was a significant positive correlation between the buckling bending drawing force and the stretching drawing force parameters, and a significant positive correlation between the rolling force back-rotation force and the rolling pre-rotation force parameters. The multivariate linear regression analysis on the parameters of influencing factors and manipulative biomechanics showed that there was a significant correlation between body weight and rolling back-rotation force, significant correlation between elbow tenderness and the buckling bending drawing force, and significant correlation between disease duration and the stretching drawing force.@*CONCLUSION@#The bachuorounian manipulation of humeral epicondylitis has a certain range of operating force. Manipulation of each stage has a correlation and systematic. The patient's weight, elbow tenderness and disease course are important factors affecting the bachuorounian manipulation.

Abstract Objective To verify if the subjective elbow value (SEV) scale presents similar results to those of the Patient-rated Tennis Elbow Evaluation (PRTEE) scale in the evaluation of patients with lateral elbow epicondylitis (LEE). Methods Thirty-seven patients were diagnosed with LEE in the outpatient service of our hospital through clinical history, physical examination, X-ray, and ultrasonography. The SEV and PRTEE scales were used and their results were compared using a significance level ≥ 5% (p ≥0.05). Results A statistically significant relationship was found between the values of SEV and PRTEE in the group of patients studied (p= 0.017). Conclusion Subjective elbow value presented similar results to PRTEE in the evaluation of patients with diagnosis of LEE.


Resumo Objetivo Avaliar se a aplicação das escalas subjective elbow value (SEV) e Patient-rated Tennis Elbow Evaluation (PRTEE) apresentam resultados similares na avaliação de pacientes com epicondilite lateral do cotovelo. Métodos Trinta e sete indivíduos com diagnostic de epicondilite lateral do cotovelo foram avaliados no ambulatório de cirurgia do ombro e cotovelo do nosso hospital. O diagnóstico foi realizado com a história clínica da patologia, exame físico, raio-x, e ultrassonográfia. Foram utilizadas as escalas SEV e PRTEE, e os resultados foram comparados estatisticamente, usando-se como nível de significância 5% (p ≥ 0,05). Resultados Encontramos uma relação estatisticamente significante entre os valores obtidos pelas escalas SEV e PRTEE quando aplicadas no grupo de pacientes portadores de epicondilite lateral (p= 0,017). Conclusão Subjective elbow value apresentou resultados similares ao PRTEE na avaliação de pacientes com diagnóstico de epicondilite lateral do cotovelo.

ABSTRACT

PURPOSE: The purpose of this study was to investigate the muscle activities of the brachioradialis and extensor carpi radialis longus according to the type of backhand stroke in badminton.METHODS: To measure the muscle activities, we used electromyography (EMG) equipment to measure EMG values by performing maximal voluntary contraction (MVC) of the forearm muscles, which depends on the type of backhand stroke. With these values, the %maximum voluntary isometric contraction values were obtained. The data were calculated using SPSS ver. 21.0 and one-way repeated measures analysis of variance with a post-hoc least significant difference test.RESULTS: In this study, the backhand clear achieved higher maximum muscle activity values than those by the backhand push, under clear, and drive in the brachioradialis. The backhand smash achieved higher maximum muscle activity values than those by the backhand push in the extensor carpi radialis longus.CONCLUSION: The backhand clear is associated with a higher injury rate than those associated with the backhand push, under clear, and drive. The backhand smash is associated with a higher rate of the occurrence of tennis elbow than that associated with the backhand push. To prevent injuries, it is considered that the forearm's strength, stretching before and after exercise, and sufficient time to rest are important.

Resumen: Objetivo: Mostrar el resultado a corto plazo de la liberación artroscópica en pacientes que presentan epicondilitis crónica lateral. Material y métodos: Se realiza liberación artroscópica de tres pacientes con epicondilitis lateral. Seguimiento de seis meses. Se utiliza la escala de Clínica Mayo para valoración de resultados. Se lleva a cabo revisión y discusión de la literatura. Resultados: Dos pacientes del género femenino y uno del género masculino dedicados a las actividades cotidianas, no deportistas. El dolor fue el síntoma capital que afectó el puntaje de la escala de valoración. Estos puntajes mejoraron luego de la cirugía. Se logró retorno precoz a sus actividades cotidianas. No se reportaron complicaciones neurológicas. Discusión: El tratamiento artroscópico resultó una alternativa segura y eficaz para el tratamiento de la epicondilitis lateral en estos tres pacientes, el cual permite realizar simultáneamente exploración articular con fines diagnósticos y tratar patologías asociadas. Se requieren series más amplias y estudios comparativos a fin de establecer protocolos definitivos en nuestra casuística.


Abstract: Objective: To assess the outcome of arthroscopic release in three patients with chronical lateral epicondylitis. Material and methods: Arthroscopic release in three patients with lateral epicondylitis is performed. Mayo Clinic scale for evaluation of results is used. A review and discussion of the literature is made. Results: Three patients, two female and one male, the common activities was principal labors, not athletes. Patients had significant pain. It was the principal symptom that affect the score of the rating scale. These scores improved after surgery. It was achieved early return to normal daily activities. No neurological complications were reported. Discussion: Arthroscopic treatment was an alternative safe and effective for treating chronical lateral epicondilitis in this three cases. It allows simultaneous joint exploration for diagnostic purposes and to treat associated pathologies. Broader Series and studies are necessary in order to establish definitive protocols in our cases.

OBJECTIVE@#To evaluate clinical efficaly of intractable lateral epicondylitis by extracurricular arthroscopic operation based on pressure point.@*METHODS@#From October 2015 to September 2017, 19 patients with intractable lateral epicondylitis were treated with extraarticylar arthroscopic operation based in pressure point. Among patients, including 7 males and 12 females, aged from 33 to 62 years old with an average of(43.16±8.12) years old, The courses of conservative treatment ranged from 7 to 41 months, with an average of(15.47±7.08) months. Postoperative complications were observed, VAS score and Mayo score before and after operation at 3 months were observed and compared.@*RESULTS@#All patients were followed up from 6 to 26 months, with an average (17.16±5.25) months. No infection, skin necrosis and nerve injury occurred. No group weakness occurred within six months after operation. VAS score decreased from 4.42±1.17 before operation to 0.53±0.61 after operation at 3 months. Mago was improved from 62.63±7.88 before operation to 93.42±5.28 after operation at 3 months. According to Mayo score, 17 cases got excellent results, and 2 cases were good.@*CONCLUSIONS@#Intractable lateral epicondylitis by arthroscopic extracurricular operation based on pressure point, which deal with main extracurricular root cause, could anatomical level is understand easily, field of vision is good and diseased tissue is cleaned up thoroughly, and has obvious curative effect.

ABSTRACT

PURPOSE: The aim of this study was to examine the prevalence of upper extremity musculoskeletal (MSK) diseases and to identify factors influencing disability among fruit tree farmers in Korea. MATERIALS AND METHODS: Of the 1150 subjects of the Namgaram study, 460 fruit tree farmers completed a questionnaire and underwent clinical evaluations, including physical assessments, laboratory tests, simple radiographic examinations, and magnetic resonance imaging studies of the upper extremities. Disability was assessed using the Disabilities of the Arm, Shoulder, and Hand outcome measure. Data were analyzed with descriptive statistics and regression analyses using SPSS Win 24.0. RESULTS: The prevalences of upper extremity MSK diseases were 60.4% for rotator cuff tear, 20.9% for golf elbow, 40.9% for tennis elbow, and 58.0% for hand osteoarthritis. Disability in fruit farmers was associated with female sex (B=−4.47, p<0.001), smoking (B=−4.00, p=0.026), depression (B=2.83, p<0.001), working hours (B=0.96, p=0.001), injuries of the arms (B=10.78, p<0.001) and shoulders (B=6.75, p<0.001), and numbers of upper extremity MSK diseases (B=2.02, p=0.001), with 26.5% of the variance explained (R2=0.265, Durbin-Watson test=1.81, p<0.001). CONCLUSION: Fruit tree farmers remain at risk for MSK diseases of the upper extremities. Disability tended to worsen with more MSK diseases. It is necessary to not only educate farmers about prevention strategies, but also to develop an effective management system for agricultural work-related MSK diseases and a surveillance system at the government level for the health problems of farmers.

ABSTRACT

Background@#Lateral epicondylalgia (LE) is a cumulative strain injury affecting the common extensor origin of the elbow, manifesting as lateral elbow pain. Tightness of the fascia connecting the lateral elbow area with the shoulder area was assumed as potential source of LE. Limitation in shoulder rotatory motions may be associated with painful LE elbows.@*Aim@#To determine the difference on shoulder rotatory motions between sides of symptomatic and asymptomatic elbows.@*Methods@#Eligible participants had at least one elbow that tested positive for Cozen, Mill, or Maudsley’s test. Using a universal goniometer, a blinded assessor measured the participants’ active and followed by passive shoulder internal and external rotation. The primary investigator tested the external rotation followed by internal rotation of the right upper extremity, then subsequently the left upper extremity of healthy participants both passively and actively@*Results@#The assessor showed excellent intra-tester reliability in measuring active and passive shoulder rotatory motions of 20 asymptomatic right upper extremities (ICC=0.98). Twenty-seven (27) participants (3 males, 24 females) with a mean (95%CI) age of 54 (49-58) years old were enrolled in the study. The mean visual analogue scale of the patients was 6.53 (5.91- 7.13), with mean (95%CI) duration of 96 (50-142) weeks. Based on hand dominance and side of LE, significant difference was found in active and passive shoulder internal rotation (p>0.05). @*Conclusion@#Shoulder active and passive internal rotations were significantly associated with hand dominance in patients with LE. Tightness of the fascia and muscle in the shoulder and painful LE elbow may underpin the decreased shoulder rotatory motions.

ABSTRACT

Extracorporeal shock waves are unique sound waves that generate a rapid increase in pressure in the tissues in which they are applied. They were first used on the management of urolithiasis and later spread to other pathologies such as musculoskeletal. A series of studies have been carried out to support the usefulness of shock waves in patients with plantar fasciitis, Achilles tendinopathy, epicondylitis, rotator cuff injury with or without calcification, patellar tendinopathy and peritrochanteric pathology that has lasted more than three months and have not responded to the initial handling. There is no consensus regarding the specific protocols to be used. In these conditions, shock waves are only a part of a comprehensive treatment, and should always be associated with other conventional therapies. (AU)

ABSTRACT

La epicondilalgia lateral es una condición músculoesquelética bastante común, que se caracteriza por la presencia de dolor, disminución en la fuerza de prensión y pérdida progresiva de la funcionalidad de la mano. La acupuntura es un método terapéutico derivado de la medicina china tradicional, en donde se realiza estimulación de puntos corporales mediante agujas, con fines terapéuticos o preventivos. Algunos estudios han reportado que la acupuntura produce un beneficio a corto plazo sobre el dolor localizado en el epicóndilo lateral del codo; sin embargo, los beneficios que informan los estudios asociados a esta patología no logran ser concluyentes. Objetivo: resumir la evidencia existente en revisiones Cochrane y no Cochrane, buscando los efectos y beneficios de la acupuntura para el tratamiento del dolor en sujetos que presenten epicondilalgia lateral. Materiales y métodos: se realizó una búsqueda en el registro especializado Cochrane del grupo músculo-esquelético (1980-junio 2015) y en las bases de datos: Medline (1980-septiembre 2015), Pedro (1980- septiembre 2015), Metabuscador Epistemonikos (1980- septiembre 2015), se buscó literatura gris y actas de congresos. La calidad metodológica de las revisiones sistemáticas incluidas fueron evaluadas con AMSTAR y la calidad global de la evidencia mediante GRADE. Resultados: La búsqueda preliminar identificó 92 revisiones sistemáticas, de las cuales 2 cumplieron con los criterios de elegibilidad. Las 2 revisiones sistemáticas fueron categorizadas como de alta calidad metodológica. Conclusión: a pesar de lo poco común de la prescripción y su aplicación en sujetos que presentan epicondilalgia lateral, la acupuntura ha demostrado ser eficaz en el alivio del dolor a corto plazo


Introduction: Epicondylalgia lateral skeletal muscle is a fairly common condition, characterized by the presence of pain, decreased grip strength and progressive loss of function of the hand. Objective: To summarize the evidence in Cochrane and non-Cochrane, looking at the effects and benefits of acupuncture for the treatment of pain in patients who may be le. Methods: A search was conducted in the specialized registry of the Cochrane musculoskeletal group (1980- September 2015) and databases: Medline (1980- September 2015), Pedro (1980- September 2015), Metasearcher Epistemonikos (1980- September 2015), gray literature, conference proceedings was sought. The methodological quality of systematic reviews was evaluated including AMSTAR and the overall quality of the evidence using GRADE. Results: The search identified 92 preliminary systematic reviews, of which two met the eligibility criteria. 2 systematic reviews were categorized as high methodological quality. Conclusion: Despite the rarity of prescription and their application in subjects with lateral epicondylalgia, acupuncture has been proven effective in relieving pain in the short term

A acupuntura é um método terapêutico derivado da medicina chinesa tradicional, onde se realiza estimulação de pontos corporais mediante guias, com fins terapêuticos ou preventivos. Alguns estudos têm reportado que a acupuntura produz um benefício a curto prazo sobre a dor localizada no epicôndilo lateral do cotovelo; no entanto, os benefícios que informam os estudos associados a esta patologia não conseguem ser concluintes. Objetivo: Resumir a evidência existente em revisões Cochrane e não Cochrane, buscando os efeitos e benefícios da Acupuntura para o tratamento da dor em sujeitos que apresentem Epicondilalgia lateral. Materiais e métodos: Realizou-se uma busca no registro especializado Cochrane do grupo musculoesquelético (1980-Junho 2015) e nas bases de dados: Medline (1980-Setembro 2015), PEDro (1980- Setembro 2015), Metabuscador Epistemonikos (1980- Setembro 2015), se buscou literatura cinzenta e atas de congressos. ABSTRACT OBJECTIVE: To evaluate the results of patients undergoing arthroscopic surgical treatment of refractory lateral epicondylitis, identifying poor prognosis factors. METHODS: A retrospective study of 44 patients (47 elbows) who underwent arthroscopic debridement of the extensor carpi radialis brevis (ECRB) tendon to treat refractory lateral epicondylitis from February 2013 to February 2015, operated by a single surgeon at one center. Patients were assessed by DASH score, visual analog scale of pain (VAS), and ShortForm 36 (SF-36). The mean age at surgery was 44.4 years (32-60). The duration of symptoms prior to the surgery was approximately 2.02 years (range: 6 months to 10 years). Mean follow-up was 18.6 months (range of 6-31.9) . RESULTS: The mean postoperative DASH score was 25.9 points; mean VAS, 1.0 point at rest (all the patients with mild pain) and 3.0 points at activity, of which 31 (66%) cases presented mild pain, 10 (21%) moderate pain, and six (13%) severe pain; mean SF-36 score was 62.5. A moderate correlation was observed between duration of pain before surgery and the DASH score with the final functional outcome. No significant complications with the arthroscopic procedure were observed. CONCLUSIONS: Arthroscopic surgical treatment for recalcitrant lateral elbow epicondylitis presented good results, being effective and safe. The shorter the time of pain before surgery and the lower the preoperative DASH score, the better the prognosis.


RESUMO OBJETIVO: Avaliar os resultados dos pacientes submetidos a tratamento cirúrgico artroscópico da epicondilite lateral refratária a tratamento conservador e identificar fatores de pior prognóstico. MÉTODOS: Estudo retrospectivo de 44 pacientes (47 cotovelos) submetidos a desbridamento cirúrgico artroscópico do tendão extensor radial curto do carpo (ERCC) para tratamento de epicondilite lateral refratária a tratamento conservador de fevereiro de 2013 a fevereiro de 2015, operados por um único cirurgião em um único centro. Os pacientes foram avaliados pelo escore de DASH, pela classificação visual analógica de dor (EVA) e pelo Short-Form 36 (SF-36). A média de idade na cirurgia foi de 44,4 anos (32 a 60). O tempo de sintomas antes da cirurgia foi de 2,02 anos (variação de seis meses a 10 anos). O seguimento médio foi de 18,6 meses (variação de seis a 31,9) . RESULTADOS: A média dos escores pós-operatórios foi de 25,9 pontos no DASH; 1 ponto no EVA de repouso (todos os casos de dores leve) e 3 pontos na EVA em atividade, 31 (66%) casos de dores leves, 10 (21%) de moderadas e seis (13%) de intensas; SF-36 de 62,5. Observou-se uma correlação moderada entre o tempo de dor antes da cirurgia e a pontuação no escore de DASH com o resultado funcional final. Não foram observadas complicações significativas com o procedimento por via artroscópica. CONCLUSÕES: O tratamento cirúrgico artroscópico para epicondilite lateral recalcitrante do cotovelo apresenta bons resultados, é eficaz e seguro. Quanto menor o tempo de dor antes da cirurgia e quanto menor o DASH pré-operatório, melhor o prognóstico.

ABSTRACT

The purpose of this study is to clinically evaluate and report the effectiveness of radiofrequency microdebrider (Topaz, ArthroCare) treatment in lateral epicondylitis patients. From March to July 2003, 15 patients of 17 elbows were prospectively followed. Candidate for the treatment were lateral epicondylitis patients who had symptom more than 6 month and failed to respond to conservative treatment, including medication (non-steroidal anti-inflammatory drugs), external gel or patch, and steroid injections. All patient who scored greater than grade 3 (fair) on the Self-administered Roles and Maudsley Pain (SRMP) score were selected for the procedure. All procedure was done using local anesthesia and ArthroCare microdebrider by a single surgeon. Postoperative assessments were done on postoperative period 12 month of two previous subjective scores and a simple functional assessment asking better, same, or worst function after the procedure. Mean age of the patients was 45 years old. Mean symptom duration before the procedure was 22.6 months. After the procedure, the mean Pain Visual Analogue Scale improved from 7.3 (range, 5–9; standard deviation [SD], 1.2) preoperatively to 3.7 (range, 0–7; SD, 2.1) postoperatively (p< 0.001). After the procedure, five elbows showed no rating improvement, in seven elbows 1 level improvement, in four elbows 2 level improvement, and in one elbow 3 level improvement. Overall, 71% (12/17) showed improvement after the procedure according to the SRMP score rating. Although 29% (5/17) of the elbow showed no improvement on SRMP score, among them five elbows were still rated decrease in Pain Visual Analogue Scale.

ABSTRACT

The natural course of lateral epicondylitis is widely regarded to be self-limiting within 1 year of symptom duration in 90% of all patients. The spectrum of treatments include simple ‘wait and see', bracing, physical therapy, corticosteroid injection, and recently some biologic injection such as autologous blood and platelet rich plasma. However, recalcitrant lateral epicondylitis which are not responding to conservative treatments can be treated with surgical treatment although much remains unclear regarding the ideal treatment. Debates surrounding open procedures are the management for the defect after resection of pathologic tissue, necessity of decortication, selective denervation procedure, etc. Since the trend is changing to minimal invasive surgery and arthroscopic release of extensor carpi radialis brevis tendon are becoming more popular these days, surgical tips and pitfall to obtain good results and avoid complications have been reported recently. Bipolar radiofrequency (RF)-based microtenotomy or percutaneous tendon release is another surgical procedures reported to be effective in lateral epicondylitis. However, there are some patients who present with persistent pain after surgical treatment. Thus, selection of ideal candidates for surgery, thorough evaluation of all pathologies prior to surgery, and adequate surgical procedures would be essential in the surgical treatment of lateral epicondylitis.

To evaluate the effects of platelet-rich plasma (PRP) infiltration in patients with lateral epicondylitis of the elbow, through analysis of the Disabilities of the Arm, Shoulder and Hand (DASH) and Patient-Rated Tennis Elbow Evaluation (PRTEE) questionnaires. METHODS: Sixty patients with lateral epicondylitis of the elbow were prospectively randomized and evaluated after receiving infiltration of three milliliters of PRP, or 0.5% neocaine, or dexamethasone. For the scoring process, the patients were asked to fill out the DASH and PRTEE questionnaires on three occasions: on the day of infiltration and 90 and 180 days afterwards. RESULTS: Around 81.7% of the patients who underwent the treatment presented some improvement of the symptoms. The statistical tests showed that there was evidence that the cure rate was unrelated to the substance applied (p = 0.62). There was also intersection between the confidence intervals of each group, thus demonstrating that the proportions of patients whose symptoms improved were similar in all the groups. CONCLUSION: At a significance level of 5%, there was no evidence that one treatment was more effective than another, when assessed using the DASH and PRTEE questionnaires.


Avaliar os efeitos da infiltração do plasma rico em plaquetas (PRP) em pacientes com epicondilite lateral do cotovelo (ELC) pela análise dos questionários Deficiência do Braço, Ombro e Mão (Dash) e Avaliação do Paciente Portador do Cotovelo de Tenista (PRTEE). MÉTODOS: Foram randomizados e avaliados prospectivamente 60 pacientes, após receberem infiltrações de três mililitros de PRP, ou neocaína 0,5%, ou dexametasona. Para o processo de pontuação, os pacientes foram convidados a preencher os questionários Dash e PRTEE em três ocasiões: no dia da infiltração, 90 e 180 dias após. RESULTADOS: Dos pacientes submetidos ao tratamento, 81,7% apresentaram melhoria dos sintomas. Os testes estatísticos demonstraram que há evidências de que a taxa de cura não está relacionada com a substância aplicada (p = 0,62). Houve também interseção dos intervalos de confiança de cada grupo, com demonstração de que as proporções de pacientes cujos sintomas melhoraram foram semelhantes em todos os grupos. CONCLUSÃO: Em um nível de significância de 5%, não houve evidência de que um tratamento foi mais eficaz do que a outro, quando avaliados pelos questionários Dash e PRTEE.

Lateral epicondylitis is a common cause of pain and functional impairment of the elbow. Usually is due to the repetition not only of certain sporting gestures but also of repetitive working activities. The vast majority of cases resolve through nonsurgical treatments. Antecedentes: La epicondilitis es una patología con una alta frecuencia en la población mexicana, estudios recientes refieren buenos resultados con plasma rico en plaquetas. Sin embargo, el tratamiento más comúnmente usado en México es la aplicación de corticoesteroide. Objetivo: Realizar en estudio comparativo en donde se determine qué tratamiento tiene mejores resultados (plasma rico en plaquetas vs corticoesteroide). Material y métodos: Se conformaron dos grupos de personas aleatorizadas haciendo dos grupos, el grupo A fue tratado con una sola aplicación de plasma rico en plaquetas y el grupo B fue tratado con 40 mg de metil prednisolona, dosis única. Se realizó una evaluación con la escala visual análoga y con la escala Quick DASH, dicha prueba fue aplicada previo al procedimiento, al mes, a los tres y seis meses. Resultados: Los resultados fueron comparados con un análisis de t de Student. Al mes de la evaluación, los pacientes muestran mejoría sin diferencia significativa. A los tres meses, el plasma rico en plaquetas muestra mejores resultados; sin embargo, a los seis meses se ve un discreto decremento de ésta. Conclusiones: De acuerdo con los resultados obtenidos, el plasma rico en plaquetas muestra mejores resultados en cuanto mejoría del dolor y duración del tratamiento; sin embargo, aún está pendiente definir el número de aplicaciones, así como la posibilidad de obtener mejores resultados al acompañarlo de rehabilitación.


Background: Epicondylitis is a highly frequent condition in the Mexican population. Recent studies have reported good results with platelet rich plasma. However, corticosteroids are the most common treatment used in Mexico. Objective: To conduct a comparative study to determine which treatment provides better results (platelet rich plasma versus corticosteroids). Material and methods: Patients were randomized into two groups. Group A was treated with a single administration of platelet rich plasma and group B with a single dose of 40 mg of methylprednisolone. Patients were assessed using a visual analog scale (VAS) and the Quick DASH scale, applied before the procedure, and at one, three and six months. Results: The results were compared with an analysis using the Student t test. One month after treatment the patients had improvement with no significant difference between both groups. At three months results were better with platelet rich plasma; however, at six months improvement with the latter declined slightly. Conclusions: The results show that platelet rich plasma provided better results from the perspective of pain control and effect duration. However, the number of applications and the possibility of improving the outcomes if combined with rehabilitation have not yet been determined.

ABSTRACT

Se realizó una intervención terapéutica en 20 pacientes con epicondilitis, remitidos de la consulta de Ortopedia al Departamento de Medicina Natural Tradicional del Hospital Provincial Docente Clinicoquirúrgico "Saturnino Lora Torres" de Santiago de Cuba, de septiembre a diciembre de 2012, con vistas a determinar la efectividad de la acupuntura con tachuela en ellos. Los afectados fueron valorados y, de forma aleatoria, se les distribuyó en 2 grupos de 10 integrantes cada uno: el de estudio, que recibió acupuntura (agujas de 0,2 mm) de forma permanente en los puntos Ashi y en el IG-11, y el de control, tratado convencionalmente con antiinflamatorios y analgésicos. Luego de aplicadas las terapias, los pacientes fueron valorados a los 5, 10 y 15 días, teniendo en cuenta el cuadro clínico dado por dolor, inflamación e impotencia funcional. La regresión de los síntomas se evaluó por la escala visual analógica, y se obtuvo una efectividad marcada del tratamiento con acupuntura en relación con el medicamentoso.


A therapeutic intervention in 20 patients with epicondilytis, transferred from the Orthopedics Department to the Traditional Natural Medicine Department from "Saturnino Lora Torres" Teaching Clinical Surgical Provincial Hospital in Santiago de Cuba was carried out from September to December of 2012, aimed at determining the effectiveness of the acupuncture with tacks in them. Those affected were evaluated and, were randomly distributed into 2 groups of 10 members each: the study group which received acupuncture (needles of 0.2 mm) in a permanent way in the points Ashi and in the IG-11, and the control group, conventionally treated with anti-inflammatory and analgesic drugs. After the therapies were applied, patients were evaluated after 5, 10 and 15 days, keeping in mind the clinical pattern manifested by pain, inflammation and functional impotence. The regression of the symptoms was evaluated by the analogical visual scale, and a marked effectiveness of the treatment was obtained with acupuncture in relation to the drug treatment.

ABSTRACT

<p><b>OBJECTIVE</b>To compare therapeutic effects of local injection with autoallergic platelet rich plasma (PRP) or autoallergic whole blood (AWB) for the treatment of chronic tennis elbow.</p><p><b>METHODS</b>From January 2011 to January 2014, 40 patients with chronic tennis elbow were divided into 2 groups, 20 cases in each group: PRP group and AWB group. There were 20 patients in PRP group treated with local injection of autoallergic platelet rich plasma, including 5 males and 15 females, with an average age of (47.50 ± 9.86) years old; and the average course of disease was (4.67 ± 3.27) months. Among the 20 patients in AWB group treated with local injection of autoallergic whole blood, 3 patients were male and 17 patients were female, with an average age of (46.50 ± 9.96) years old;and the average course of disease was (4.53 ± 2.27) months. The elbow joint was fixed with elastic stockings after injection. All the patients were guided to do strengthening and extension exercises during the follow-up period. Visual analog scale (VAS), Mayo scores for elbow and pressure pain threshold (PPT) were used to evaluate clinical effects after injection immediately and 4,8 weeks after treatment. Results:All the patients were followed up,there were no infections and swelling occurred. The VAS, Mayo and PPT scores of patients in PRP group were improved from pre-therapy 7.22 ± 1.32, 56.71 ± 10.90 and 17.47 ± 4.62 to 2.73 ± 1.00, 91.59 ± 6.95 and 21.35 ± 4.80 respectively 8 weeks after treatment. The VAS, Mayo and PPT scores of patients in AWB group were improved from pre-therapy 7.16 ±1.27, 54.72 ± 8.36 and 17.06 ± 4.83 to 3.81 ± 1.36, 82.06 ± 7.89 and 20.12 ± 4.97 respectively 8 weeks after treatment. All the pain and functional variables including VAS, PPT, and Mayo scores were improved significantly in both groups 4 weeks after injection. On the 4th week after injection, there was no statistically significant difference in PPT between two groups; while the VAS and Mayo score of AWB group were lower than those of PRP group. On the 8th week after injection, the VAS of AWB group was higher than that of PRP group; but the Mayo and PPT scores of AWB group were lower than those of PRP group.</p><p><b>CONCLUSION</b>PRP and AWB injections are both effective to treat chronic lateral epicondylitis. Compared with AWB injection, PRP injection may be more effective in releasing pain and improving function for a longer time.</p>
